The materials contain the five forms that are to be completed as part of the assessment process, including a: (1) family interview form for a complete social history of the child; (2) parent questionnaire for identification of information areas; (3) resource utilization checklist for identification of required community services; (4) family needs inventory for staff review of family needs; and (5) individual family plan for development of a contract between staff and family to set goals and plan activities. Each section of the assessment process is explained with the help of a sample case summary. The manual is written from a social service perspective and is meant to be self-explanatory to helping professionals. Sample forms are also included along with a list of references and resources.
