520 |a The five-step family assessment process presented in this manual is designed to facilitate the accurate, organized collection of information necessary for the development of an intervention plan for families with children. The materials contain the five forms that are to be completed as part of the assessment process, including a: (1) family interview form for a complete social history of the child; (2) parent questionnaire for identification of information areas; (3) resource utilization checklist for identification of required community services; (4) family needs inventory for staff review of family needs; and (5) individual family plan for development of a contract between staff and family to set goals and plan activities. Each section of the assessment process is explained with the help of a sample case summary. The manual is written from a social service perspective and is meant to be self-explanatory to helping professionals. Sample forms are also included along with a list of references and resources.
